Ashwagandha has been used in Ayurvedic medicine for centuries, but the modern clinical research is what's made it one of the most popular supplements for stress in recent years. The evidence is real in several areas. It's also more complicated than the marketing usually suggests.

The 2025 Meta-Analysis: What It Found

A 2025 systematic review and meta-analysis published in PMC searched PubMed, Web of Science, Scopus, and Cochrane from inception through September 2024. Fifteen randomized controlled trials involving 873 adults met the inclusion criteria. All participants had confirmed stress or anxiety at baseline.

The results were statistically significant across three outcomes:

  • Cortisol levels dropped meaningfully compared to placebo
  • Perceived Stress Scale (PSS) scores improved significantly
  • Hamilton Anxiety Scale (HAM-A) scores improved significantly

Quality of life scores, however, did not show a statistically significant improvement. The authors concluded that ashwagandha is safe and effective for reducing stress and anxiety in adults, with the caveat that quality of life outcomes need more study.


The Cortisol Question: Where It Gets Interesting

A separate 2025 meta-analysis of 8 RCTs in 488 adults focused specifically on the relationship between cortisol levels and perceived stress. It found something worth understanding: ashwagandha produced a statistically significant reduction in cortisol levels (mean reduction of -1.16 µg/dL, p < 0.001), but did not produce a statistically significant reduction in perceived stress scores in this particular analysis.

That split finding is worth sitting with. Your cortisol numbers can change while your subjective sense of feeling stressed doesn't move as much. The two don't always track together, partly because perceived stress is influenced by a lot of factors beyond cortisol, and partly because the trials in the second meta-analysis were mostly conducted in India, which may limit how well the findings transfer to other populations.

The 15-trial PMC analysis did find significant PSS improvements, so the picture across the full literature is that ashwagandha reduces both cortisol and perceived stress, but the effect on perceived stress is less consistent than the cortisol finding.


How Ashwagandha Works

  • Athletic performance — there is some evidence for strength and recovery, but that's a separate body of research not the focus of the stress/cortisol literature

Dose and Form
